29 CFR 1910.269(c)(2): The employer did not ensure that the required briefing covered at least the following subjects: hazards associated with the job, work procedures involved, special precautions, energy-source controls, and personal protective equipment requirements: NEW LONDON On, or about, July 22, 2015, the employer did not ensure that the required job briefing included hazards associated with the job and any required special precautions, when employees were exposed to sulfur hexafluoride, and associated by-products, including, but not limited to, sulfur fluoride, sulfur oxide gases, hydrofluoric acid and sulfuric acid generated from high temperatures, such as those experienced during an arc-flash associated with a damaged Vista Switch gear box.
