29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. On or about June 3, 2015, at the West Side of the first level rooftop, an employee installing roof shingles was exposed to a 14 feet 9 inches fall hazard to a ground level, without the use of personal fall arrest system, safety net system or guardrail system. b. On or about June 3, 2015, at the West and North Side of second level rooftop, an employee installing roof shingles was exposed to a 21 feet 3 inches fall hazard to a ground level, without the use of personal fall arrest system, safety net system or guardrail system. c. On or about June 3, 2015, at the East Side of the second level rooftop, an employee installing roof shingles was exposed to about 7 feet fall hazard to a first level canopy rooftop, without the use of personal fall arrest system, safety net system or guardrail system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On or about June 03, 2015, at west side of the residence, three employees were exposed to a fall hazard when ascending to a rooftop on a ladder which was not extended at least 3 feet above the upper landing surface due to their length. The ladder was not secure at its top and a grasping device was not provided.
