At a residential townhome home construction site, located at 3977 Mezzanine Lane, Jacksonville, Florida: On or about September 24, 2024, the employer exposed employees to fall and ladder collapse hazards in that the extension ladder used for roof access was set at a shallow angle of repose with the horizontal distance from the top support to the foot of the ladder at approximately one-half of the working length of the ladder.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids, caustic liquids, or chemical gases or vapors and at times prior thereto, workers were exposed to a struck by hazard while cutting concrete with a saw, without the use and wear of protective eyewear: a. At a residential townhome home construction site, located at 3977 Mezzanine Lane, Jacksonville, Florida: On or about September 24, 2024, the employer exposed employees to eye injury hazards hazards, in that protective eyewear was not utilized by employees while using nail guns and metal hammers to install roofing shingles. Mediterranean Construction & Roofing, Inc. was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard CFR 1926.102(a)(1), which was contained in OSHA inspection number 1567432, Citation number 2, Item number 1, and was affirmed as a Final Order on December 27, 2022, with respect to a workplace located at 5732 Spring Park Road, Jacksonville, FL.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): Each Employee on a residential roof, with unprotective sides and edges, is not protected from falling by a personal fall arrest system. The employer does not require the use of any form of fall protection during roofing work. This violation was most recently observed on September 24, 2024, at 3977 Mezzanine Lane, Jacksonville, Florida , where two employees were not protected from fall hazards of 23 feet to the ground. To abate this violation, the employer must ensure that it's employees performing framing work are protected from falls, and that no employee is allowed to work at or above six feet high without adequate fall protection. Mediterranean Construction & Roofing, Inc. was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13 ), which was contained in OSHA inspection number 1647596, Citation number 1 Item number 1a, and was affirmed as Final Order on May 23, 2023, with respect to a workplace located at 143 Clearview Drive, Saint Augustine, FL.
