On the first floor roof of a new residential home, at a temporary worksite located at 6848 Friendship Fountain Ave, in Jacksonville, FL: On October 2, 2023, the employer exposed employees to fall hazards, in that they were not provided with a ladder to exit the first floor roof while working on the first floor roof.

29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  a.  On the front and rear sides of the building, located at 6848 Friendship Fountain Ave, Jacksonville, Florida: Mediterranean Construction & Roofing, Inc. failed to ensure that frequent and regular inspections by a competent person took place at this jobsite, as demonstrated on October 2, 2023 when three employees installing roofing materials on a 6:12 pitched roof of a two-story residence were exposed to the hazard of a 6 to 20-foot fall, in that they were not utilizing a fall protection system that would have prevented them from falling from the roof.  Mediterranean Construction & Roofing, Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1567432, citation number 1, item number 1 and was affirmed as a final order on 12/27/2022, with respect to a workplace located at 5732 Spring Park Road, Jacksonville, Florida.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a. Employees installing roofing materials at elevations of six-foot or more above the ground are not being protected by a fall protection system: The employer fails to assure effective protection against falls as required by the standard, as demonstrated on October 2, 2023, when three employees installing roofing materials on a 6:12 pitched roof of a two-story residence located at 6848 Friendship Fountain Ave in Jacksonville, Florida 32217, were exposed to the hazard of a 6 to 20-foot fall, in that they were not utilizing a fall protection system that would have prevented them from falling from the roof.  Mediterranean Construction & Roofing, Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1567432, citation number 2, item number 2 and was affirmed as a final order on March 17, 2022, with respect to a workplace located at 5732 Spring Park Road, Jacksonville, Florida.  Mediterranean Construction & Roofing, Inc.  was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1647596, citation number 1, item number 1 and was affirmed as a final order on April 10, 2023, with respect to a workplace located at 143 Clearview Drive, Saint Augustine, FL.
