29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): a. On the front and rear sides of the building, located at 6848 Friendship Fountain Ave, Jacksonville, Florida: Mediterranean Construction & Roofing, Inc. failed to ensure that frequent and regular inspections by a competent person took place at this jobsite, as demonstrated on October 2, 2023 when three employees installing roofing materials on a 6:12 pitched roof of a two-story residence were exposed to the hazard of a 6 to 20-foot fall, in that they were not utilizing a fall protection system that would have prevented them from falling from the roof. Mediterranean Construction & Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1567432, citation number 1, item number 1 and was affirmed as a final order on 12/27/2022, with respect to a workplace located at 5732 Spring Park Road, Jacksonville, Florida.

Employees installing roofing materials at elevations of six-foot or more above the ground are not being protected by a fall protection system: The employer fails to assure effective protection against falls as required by the standard, as demonstrated on October 2, 2023, when three employees installing roofing materials on a 6:12 pitched roof of a two-story residence located at 6848 Friendship Fountain Ave in Jacksonville, Florida 32217, were exposed to the hazard of a 6 to 20-foot fall, in that they were not utilizing a fall protection system that would have prevented them from falling from the roof.
