29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  The employer did not ensure that electrical equipment was used in accordance with listing and labeling instructions in the following instances:  a) Welding Department Robotic Welding Cell #1 - A power strip was plugged into an extension cord reel, which was suspended from the power cord of a dust collector.  The power strip was used to provide power to the overhead lighting in the welding cell and an extension cord to provide power to an electric hoist.  b) Welding Department Robotic Welding Cell #2 - An extension cord reel suspended over the partition was used to provide power to overhead lighting in the welding cell and a power strip used with computer equipment.  c) Welding Department Grinding Area - An extension cord was plugged into an extension cord reel to provide power to the Milwaukee hand grinder.  d) Tool Room - A household fan was plugged into a three prong electrical adaptor that was then plugged into an outlet.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cables were used as a substitute for the fixed wiring of a structure:  a) Welding Department Robotic Welding Cell #1 - A power strip was used with an extension cord reel suspended from the power cord of an overhead light fixture.  The extension cord reel provided electric power to an extension cord used with an electric hoist and a dust collector.  b) Welding Department Robotic Welding Cell #2 - An extension cord reel suspended over the partition was used to provide power to computer equipment and a power strip that was used to provide power to an overhead light located in welding cell #2.  c) Welding Department Grinding Area - The Milwaukee hand grinder was used with an extension cord plugged into an extension cord reel.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  a)  Welding Department Grinding Area - The Milwaukee grinder was used with damaged flexible cords.  The employer did not ensure that portable cord and plug connected equipment and extension cord were visually inspected before use.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition.  a) Tool Room - Mitsui Surface Grinders, Torit Dust Collector and the Jet Finishing Machine work area, walls, floors and machine surfaces were not kept in a clean, orderly and sanitary condition, as they contained an accumulation of metal dust and metal shavings exposing employees to slip, falls and eye injuries.  b) Welding Department Exit Door - The exit doorway was not kept clear and in good repair, as the walking surface was uneven, contained loose gravel, and also contained two pieces of wood (2 x 4), exposing employees to slip, trip and fall hazards.
