29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.       (a) Production - On May 25, 2018, the employer did not ensure that specific energy control procedures were established to control hazardous energy during servicing and maintenance activities such as die setting on mechanical power press machines. The lockout tagout procedure for the Verson #58 mechanical press did not address locking out all energy sources such as the gravitational energy when performing set-up with the top ram in the elevated position.      In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.217(d)(9)(iv): Safety blocks were not used when mechanical power press die(s) were being adjusted or repaired in the press:  (a) Production - On or about May 16, 2018, the employer did not ensure that safety blocks were used to block the top die from failure when adjusting dies during set-up. An employee suffered a hand amputation after the unsecured die fell when performing set up on the Verson #58 mechanical press.   (b) Production - On May 25, 2018, the employer did not ensure that safety blocks were used to block the top ram from failure when adjusting dies during set-up on the Verson #58 mechanical press.     No abatement certification or documentation is required for this item.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence to achieve energy control by performing the following steps as required by 29 CFR 1910.147(d)(1)-(6):    1.Turn off and shut down power to the machinery,  2.Physically locate and operate energy-isolating devices in such a manner as to isolate the machine or equipment from the energy source(s),  3. Affix lockout/tagout devices to each energy isolating device,  4. Verify isolation and de-energization of machinery have been accomplished prior to starting the maintenance and/or servicing work:      a) Production - On May 25, 2018, employees were exposed to machine hazards associated with moving parts when preforming die set up on part revolution mechanical presses when such tasks were performed employees. The employer did not implement energy control application steps as the machine was not shut down or turned off to perform die setting [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving dissipation machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.        No abatement certification or documentation is required for this item.
