Polo Masonry Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1) which was contained in OSHA inspection number 192119, citation 2, item 1, and was affirmed final order on or about April 17, 2012, with respect to a workplace located at 2431 W. Belmont Ave., Chicago, IL 60618.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
