29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact. (a) On September 16, 2016, Polo Masonry Builders, Inc. did not ensure that employees wore appropriate hand protection when working with Miracle Morta-Lok Masons Hydrated Lime. Miracle Morta-Lok Masons Hydrated Lime contains calcium hydroxide and can cause skin burns. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

(a) On September 16, 2016, Polo Masonry Builders, Inc. did not ensure that employees wore safety glasses with side shields when working with Miracle Morta-Lok Masons Hydrated Lime. Miracle Morta-Lok Masons Hydrated Lime contains portland cement and can cause eye damage. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
