29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    On or about 8/4/15, employees pouring molten steel into molds were not required to use protective equipment for upper body, torso, legs and feet.  Fire retardant jackets, aprons and spats or leggings were not used.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    On or about 8/4/15, an employee shaking out castings and an employee pouring molten steel into molds were exposed to hazardous chemicals including, but not limited to, molten steel, silica and respirable dust.  These employees were not trained.    Harbor Castings, Inc. was previously cited for a violation of this occupational safety and health standard,  which was contained in OSHA inspection number 1010236, citation number 1, item number 2a and was affirmed as a final order on 7/10/2015, with respect to a workplace located at 4321 Strausser St NW, North Canton, Ohio.

29 CFR 1910.1200(h)(3)(iv): Employees had not been provided with GHS 2012 training including an explanation of the labels received on shipped containers and the workplace labeling system to be used by their employer; the safety data sheet, including the order of information, and how employees can obtain and use the appropriate hazard information:  On or about 8/4/15, the employer did not provide training or information to employees working with hazardous chemicals on the details of the new hazard communication standard which encompasses the Global Harmonization System (GHS).  Harbor Castings, Inc. was previously cited for a violation of this occupational safety and health standard,  which was contained in OSHA inspection number 1010236, citation number 1, item number 2b and was affirmed as a final order on 7/10/2015, with respect to a workplace located at 4321 Strausser St NW, North Canton, Ohio.
