29 CFR 1960.8(a): 29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm:  a) 141 East Fisher Ave: On or about 06/24/2015, an employee cutting grass was exposed to hazards of tip-over and falling while operating a zero turn radius riding lawn mower on a hill with a slope greater than 17 degrees.    b) 141 East Fisher Ave: On or about 06/24/2015, at the workplace, the employer did not ensure that Roll-Over Protective Structure (ROPS) and a seatbelt were installed on the Gravely Z 60" zero turn lawnmower model #992047 in accordance with manufacturer's instructions, to prevent injury and/or death in the event of a roll over.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  141 East Fisher Ave: A bench grinder used in the maintenance department to grind down metal had a 3/8 inch gap between the right-hand side abrasive wheel and the work rest.  The U. S. Department of Agriculture Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(a): which was contained in OSHA inspection number 544723, Notice 1, Item 2a and was affirmed as a final order on 01/16/2013, with respect to a workplace located at 4350 Cliffs Drive, Pocatello, ID 83204.

Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  141 East Fisher Ave:  On or before 6/24/2014, a refrigerator and microwave oven were energized through an extension cord.  The equipment was listed for household use and instructions directed it be plugged directly into a wall outlet.      The U. S. Department of Agriculture Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2): which was contained in OSHA inspection number 92432, Notice 2, Item 2 and was affirmed as a final order on 10/26/2011, with respect to a workplace located at 215 Hoyt flt, Avery, ID 83902
