29 CFR 1910.217(b)(1): Machine component(s) of mechanical power press(es) were not designed, secured, or covered to minimize hazards caused by breakage, or loosening and falling, or release of mechanical energy:    Milark Industries, Inc. - Mansfield, Ohio: On or about July 2, 2015, the employer failed to ensure clutch/brake components of the Stamtec N15 mechanical power press (model G1-160-S; s/n 0264) were constructed and secured to protect operators from breakage, or loosening and falling components.  An employee that operated the press following a clutch/brake replacement on the machine sustained amputation injuries due to falling machine components.
