29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A) Milark Industries, Inc., located at 25 Baird Parkway, Mansfield, OH 44903: On or about September 2, 2015, the employer failed to ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, the SMC pneumatic clamp (Model MGQL63-200) of equipment such as, but not limited to, the Sankei Welding Center # 5. Employees were exposed to caught-in-between and amputation hazards.
