29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): A) Milark Industries, Inc., located at 20 Baird Parkway, Mansfield, OH 44903: On or about June 22, 2016, the employer did not ensure that the point of operation of the AddisonMckee FM-70 RF Swager # 100054, located in Building J, was adequately guarded. One of the machine operators sustained partial amputation of two fingers on the left hand. Employees were exposed to amputation hazards while operating the machine.
