29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      a) Job site, 2620 Smallman Street, Pittsburgh, PA: On or about July 24, 2015, while working on the top of an interior room  to do roofing related work on the roof above, the employer did not ensure that an employee used fall protection systems/devices where exposed to fall hazards of approximately nine feet.

29 CFR 1926.501(b)(10):  Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:   a) Job site, 2620 Smallman Street, Pittsburgh, PA: On or about July 24, 2015, during roofing work on a roof with an approximate 3 in 12 pitch, the employer did not ensure that an employee, working on the roof to aid with tear off work, was tied off in an area where it was known to have been collapsing, exposing an employee to a fall hazard of up to approximately 20 feet.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) Job site, 2620 Smallman Street, Pittsburgh, PA: On or about July 24, 2015, during roofing work no a roof with a roof pitch determined to be over 5 in 12, the employer did not ensure that appropriate fall protection systems/devices were in use in that the employer permitted the use of warning lines and a safety monitor system while working outside the warning lines, exposing employees to fall hazards of up to approximately 20 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:      a) Job site, 2620 Smallman Street, Pittsburgh, PA: On or about July 24, 2015, during roofing work, the employer did not ensure that a ladder, used to gain access to/egress from the top of an inside lower roof area, was extended 3 feet above the landing surface or that it was tied off and a grab bar provided.
