29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: a) Jobsite, 3049 Washington Pike Bridgeville, PA 15017 - On or about October 21, 2024, two employees were installing insulation on a flat, commercial, canopy roof, approximately 11.5 feet above the ground, without proper utilization of a safety monitoring system. The safety monitor was performing roofing work which took their attention away from the other employee working at a leading edge approximately 11.5 feet above the ground.

The employer did not provide retraining to employees who demonstrated lack of knowledge and use of a safety monitoring system.
