29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:       a) Employees were exposed to eye injury while operating a pneumatic nail gun without eye protection during carpentry work. 29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:       a)  Employees were not protected against accidental contact with energized overhead power lines during carpentry work.  The overhead power lines consisted of secondary lines having a voltage rating of 120/240 volts.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigged support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:       a)  An extension ladder used by employees to gain access to the working surface was not long enough to have it's side rails extend at least 3 feet above the surface.        No abatement certification or documentation is required for this item.
