29 CFR 1910.176(b): Storage of material created a hazard: a) North billet saw - On or about 11/23/15, stainless steel billets ranging from 200 - 7000 pounds is cut on a billet saw and moved to a steel cradle: 1) The front cradle had a broken arm. 2) The leg on the flipper was about 3 inches short on the bottom. 3) Two steel cross members were loose on the back cradle. 4) A weld was broken on the front cradle. 5) There were two broken welds on the back cradle. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.141(g)(2): Employees were permitted to consume food or beverage in area(s) exposed to toxic materials: (a) Shape and Mini Mill lunch room - On or about 11/2/15, a wipe sample of a wood table surface showed a concentration of 55.2 micrograms of iron oxide per 100 square centimeters. Employees were permitted to consume food or beverage in an area exposed to toxic materials. (b) Shape and Mini Mill lunch room - On or about 11/2/15, a wipe sample of the inside refrigerator door surface showed a concentration of 3.2 micrograms of copper per 100 square centimeters, 301.5 micrograms of iron oxide per 100 square centimeters, and 10.2 micrograms of chromium per 100 square centimeters. Employees were permitted to consume food or beverage in an area exposed to toxic materials. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.1026(l)(1)(ii): The employer did not address at least the following hazards with employees exposed to hexavalent chromium: Cancer, eye irritation, and skin sensitization and 29 CFR 1910.1026(l)(1)(iii): The employer did not include chromium (VI) in the hazard communication program, established to comply with the Hazard Communication Standard, 29 CFR 1910.1200, ensure that each employee had access to labels on containers of chromium (VI) and to safety data sheets, and was trained in accordance with the requirements of the Hazard Communication Standard and 29 CFR 1910.1026: a) Billet Cut saw man - On or about 11/2/15, the north Billet cut saw men was exposed to an airborne concentration of 0.0004 mg/m3 of hexavalent chromium when cutting stainless steel billets. The employer did not address at least the following hazards with employees exposed to hexavalent chromium: Cancer, eye irritation, and skin sensitization. In addition, the employer did not include hexavalent chromium in the hazard communication program. a) Billet Cut saw man - On or about 11/2/15, the south Billet cut saw men was exposed to an airborne concentration of 0.0004 mg/m3 of hexavalent chromium when cutting stainless steel billets.
