29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes: a) On or about 10/19/16, in the Bar Blaster machine area: An employee opened the door on the Bar Blaster electrical cabinet and was using a voltmeter to troubleshoot an electrical problem "live". The employee was in close proximity to live parts at 480 volts, when an arc flash occurred, injuring the employee. The employee was not wearing appropriate hand protection to protect him from burns and shock hazards. ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.335(a)(1)(v): Employees exposed to the danger of injury to the eyes or face from electric arcs or flashes or from flying objects resulting from electrical explosion did not wear protective equipment for the eyes or face: a) On or about 10/19/16, in the Bar Blaster machine area: An employee opened the door on the Bar Blaster electrical cabinet and was using a voltmeter to troubleshoot an electrical problem "live". The employee was in close proximity to live parts at 480 volts, when an arc flash occurred, injuring the employee. The employee was not wearing an arc-rated flash suit hood OR a face shield and balaclava with an arc rating of at least 8cal/cm. squared to protect his face and neck. ABATEMENT DOCUMENTATION REQUIRED

The employee was in close proximity to live parts at 480 volts, when an arc flash occurred, injuring the employee. There was no warning sign/label on the outside of the cabinet to warn Qualified Persons of potential Arc Flash Hazards. ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition: a) On or about 10/31/16, in the Bar Blaster machine area: The blasting media used in the bar blasting is Amasteel. The blasting process produces a mixture of metal dust/grit on the floor area around the entire machine. This mixture is very slippery and is a slipping/tripping hazard. There was an excessive build-up of this material on the floor at the time of the inspection. ABATEMENT CERTIFICATION REQUIRED
