29 CFR 1910.132(a): Protective equipment was not maintained in a sanitary and reliable condition: a) On or about 09/15/16 throughout the plant; employees were exposed to burn hazards while performing the following tasks, such as but not limited to, voltage testing, working in proximity to energized electrical equipment, opening disconnects and other energized electrical circuits, when their 12 cal and 43 cal arc/flame resistant coveralls were not inspected on a regular basis to determine whether the clothing was properly washed, in need of repair or replacement. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from the implement hazard(s) created by steel rods entering and being ejected from the Medart machines: a) On or about 06/17/16 in the Medart department; employees operated the #1 Medart and were exposed to an impalement hazard when the area adjacent to the exit chute was not guard allowing them to be in proximity to where the steel rods discharged onto the inspection table. b) On or about 07/20/16 in the Medart department; employees operated the #1 Medart and were exposed to an impalement hazard when the area adjacent to the west side of the feed end was not guarded allowing access to where steel rods entered the machine. c) On or about 07/20/16 in the Medart department; employees operated the #2B Medart and were exposed to an impalement hazard when the exit chute was not guarded allowing them to be in proximity to where steel rods were discharged onto the inspection table. d) On or about 07/20/16 in the Medart department; employees operated the #5 Medart and were exposed to an impalement hazard when the guard on the east side of the feed end was not secured and easily removable allowing access to the area where steel rods enter the machine. e) On or about 07/20/16 in the Medart department; employees operated the #2A Medart and were exposed to an impalement hazard when the exit chute was not guarded allowing them to be in proximity to where the steel rods were discharged onto the inspection table. f) On or about 07/20/16 in the Medart department; employees operated the #0 Medart and were exposed to an impalement hazard when the exit chute was not guarded allowing them to be in proximity to where the steel rods were discharged onto the inspection table. NO ABATEMENT DOCUMENTATION REQUIRED
