29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): The employer does not protect each employee from the hazard of falling by the use of fall protection devices. a) This most recently occurred on August 17, 2015, at 760-770 Stonewall Ct., Schaumburg, Illinois. Employees were exposed to the hazard of falling while performing roofing work from a two story pitched roof. To abate this hazard in the future, the employer must ensure that employees are protected from the hazard of falling by the use of fall protection devices. This employer has been cited for this practice that included: Ziggys Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 573578, citation 2, item 5, and was affirmed final order on or about May 13, 2013, with respect to a workplace located at 1222 Long Valley, Palatine, IL 60067 Ziggys Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 314817230, citation 2, item 1, and was affirmed final order on or about October 13, 2010, with respect to a workplace located at 375 Irving Park Rd., Wood Dale, IL 60191. Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect its employees from fall hazards.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Employees were exposed to possible danger of head injury where protective helmets were not provided when employees were required to unload pallets of roofing materials being delivered to a roof surface by crane. Ziggy's Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100 (a), which was contained in OSHA inspection number 573578, citation 2, item 1, and was affirmed as a final order on March 16, 2013, with respect to a worksite located at 375 Irving Park Road, Wood Dale, Illinois. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
