29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:  a) On September 6, 2017, employees were using pneumatic nail guns without protective eye equipment while performing residential roofing work.  Ziggy's Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1081202, citation number 1, item number 1 and was affirmed as a final order on February 24, 2016, with respect to a workplace located at 3658 N. Linder Ave., Chicago, IL 60641.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  The employer does not protect each employee from the hazard of falling by the use of fall protection devices.  a) This most recently occurred on September 6, 2017, employees were performing roofing work without fall protection and were exposed to the hazard of falling between approximately 10 to 19 feet.  To abate this hazard in the future, the employer must ensure that employees are protected from the hazard of falling by the use of fall protection devices.  The employer has been cited for this practice that included:  Ziggy's Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1085389, citation number 1, item number 1 and was affirmed as a final order on February 24, 2016, with respect to a workplace located at 760-770 Stonewall Ct., Schaumburg, IL 60173.  Ziggy's Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1081212, citation number 2, item number 2 and was affirmed as a final order on February 24, 2016, with respect to a workplace located at 3658 N. Linder Ave., Chicago, IL 60641.  Ziggy's Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 573578, citation number 2, item number 5 and was affirmed as a final order on May 13, 2013, with respect to a workplace located at 1222 Long Valley, Palatine, IL 60067.
