No abatement certification or documentation required for this violation.

29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):        a) On or about May 9, 2022, employees were exposed to the hazard of being struck by material and debris that was being thrown from the roof deck at an elevation of approximately 25 feet where an enclosed chute or equivalent material was not provided.      No abatement certification or documentation required for this violation.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   The employer does not protect each employee from the hazard of falling by the use of fall protection devices.      a) This most recently occurred on May 9, 2022, at 5001 River Road, Schiller Park, Illinois, where employees were exposed to the hazard of falling approximately 25 feet while performing roofing work from pitched roof surfaces without the benefit of fall protection.   To abate this hazard in the future, the employer must ensure that employees are protected from the hazard of falling by the use of fall protection devices.   This employer has been cited for this practice that included:  Ziggy's Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1261655, citation 1, item 1, and was affirmed as a final order on or about November 28, 2018, with respect to a worksite located at 1604 St. Andrew, Vernon Hills, Illinois.  Ziggy's Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1261655, citation 1, item 2, and was affirmed as a final order on or about January 18, 2018, with respect to a worksite located at 904 David Lane, Elk Grove Village, Illinois.  Ziggy's Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1085389, citation 1, item 1, and was affirmed as a final order on or about May 9, 2016, with respect to a worksite located at 760-770 Stonewall Court, Schaumburg, Illinois.  Ziggy's Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1081202, citation 2, item 2, and was affirmed as a final order on or about May 9, 2016, with respect to a worksite located at 3658 N. Linder Ave., Chicago, Illinois.  Ziggy's Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 573578, citation 2, item 5, and was affirmed as a final order on or about May 13, 2013, with respect to a worksite located at 1222 Long Valley, Palatine, Illinois.  Ziggy's Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 314817230, citation 2, item 1, and was affirmed as a final order on or about October 12, 2010, with respect to a worksite located at 375 Irving Park Road, Wood Dale, Illinois.  Ziggy's Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 313147324, citation 1, item 4, and was affirmed as a final order on or about June 16, 2009, with respect to a worksite located at 6457 N. Hiawatha, Chicago, Illinois.   Pursuant to 20 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect its employees from fall hazards.
