(a) At the BLM Lake Havasu District Office, Fire Business cubicle, exit access/egress was approximately 16 inches and obstructed by trash can, boxes of materials and book shelves.  (b) At the TELECOM and ADP room, exit access/egress was approximately 26 inches and obstructed by computer materials and monitors.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  (a) At the fire maintenance officer, a re-locatable power tap was connected from an extension cord to plug in the television.  (b) At the District office employee cubicle, a re-locatable power tap was used to plug in the microwave oven and the refrigerator.  The Bureau of Land Management was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2) which was contained in OSHA inspection number 315492298, citation number 1, item number 2A and was affirmed as a final order on 8/30/2011, with respect to a workplace located at 1400 S. Front Street, Caliente, NV 89008.  The Bureau of Land Management was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2) which was contained in OSHA inspection number 315492413, citation number 1, item number 3 and was affirmed as a final order on 9/29/2011, with respect to a workplace located at 300 S. Richmond Road, Ridgecrest, CA 93555.   The Bureau of Land Management was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2) which was contained in OSHA inspection number 315681775, citation number 1, item number 4 and was affirmed as a final order on 10/27/2011, with respect to a workplace located at 1340 Financial Blvd., Reno, NV 89502.
