The employer failed to implement a monitoring program.

29 CFR 1960.66(b): Where the employer failed to comply with the requirements under 29 CFR 1904, subparts C, D, E, and G, by failing to provide copies of injury and illness records as required by Subpart E, 1904.40(a): (a) On or about July 20, 2022 for calendar years 2021, 2020, and 2019, the employer did not provide the OSHA 300A (or equivalent annual summary) and OSHA 300 logs requested by OSHA within four business hours located at 702 North Industrial Way in Ely, Nevada.
