29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers shall be equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section.   a. Located in the Assembly Department at 9000 Smith's Mill Road in New Albany, Ohio: the stairway used to access the logistics platform located adjacent to the north half of the department did not have a railing, exposing employees to a fall hazard.     b. Located in the Assembly Department at 9000 Smith's Mill Road in New Albany, Ohio: the stairway used to access the logistics platform located adjacent to the south half of the department did not have a railing, exposing employees to a fall hazard.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.     a. Located in the Assembly Department at 9000 Smith's Mill Road in New Albany, Ohio: relocatable power strips were used to supply electric to numerous belt conveyors and adjacent equipment, such as taping and labeling machines. The power strips were located at, but not limited to, assembly lines 13, 14, 16, 17, 18, 19, 24, 64 and were not approved for use, exposing employees to an electrical shock and fire hazard.      b. Located in the Assembly Department at 9000 Smith's Mill Road in New Albany, Ohio: metal boxes with double duplex receptacles were placed on the floor beneath numerous belt conveyors. The metal boxes were located at, but not limited to, assembly lines 25, 26. 56. 57, 65. The boxes were not secured in accordance with their UL listing and labeling, exposing employees to an electrical shock hazard.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    a. Located in the Assembly Department at 9000 Smith's Mill Road in New Albany, Ohio: the employer failed to adequately protect employees from the ingoing nip point located at the end roll of the conveyor at assembly line 13, exposing employees to a caught-in hazard.       b. Located in the Assembly Department at 9000 Smith's Mill Road in New Albany, Ohio: the employer failed to adequately protect employees from the ingoing nip point located at the end roll of the conveyor at assembly line 15, exposing employees to a caught-in hazard.       c. Located in the Assembly Department at 9000 Smith's Mill Road in New Albany, Ohio: the employer failed to adequately protect employees from the ingoing nip point located at the end roll of the conveyor at assembly line 55, exposing employees to a caught-in hazard.       d. Located in the Assembly Department at 9000 Smith's Mill Road in New Albany, Ohio: the employer failed to adequately protect employees from the ingoing nip point located at the end roll of the conveyor at assembly line 57, exposing employees to a caught-in hazard.    The Accel, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number965727, citation number1, item number1 and was affirmed as a final order on June 04, 2014, with respect to a workplace located at 9000 Smith's Mill Road, New Albany, OH 43054.

Located in the Assembly Department at 9000 Smith's Mill Road in New Albany, Ohio: an extension cord was used as a substitute for fixed wiring for the logistics platform located at the south section of the department.
