Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected. a. When an injury or illness involves one or more days away from work, you must record the injury or illness on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days column. If the employee is out for an extended period of time, you must enter an estimate of the days that the employee will be away, and update the day count when the actual number of days is known. Located at 9000 Smith's Mills Road, New Albany, Ohio: a. On the 2021 OSHA 300 log, the employer did not ensure that Column H was checked and did not ensure that an estimate of the days that the employee will be away from work was included in Column K. 1. On July 16, 2021, an employee was injured while operating a forklift that resulted in days away from work. The employer did not check a box in Column H of the log to classify the injury nor did the employer include an estimate in Column K of the days that the employee will be away from work.

Located at 9000 Smith's Mills Road, New Albany, Ohio: a. On the 2021 OSHA 300 log, the employer did not ensure that Column F included the necessary description of the injury on the log. 1. On July 16, 2021, an employee was injured while operating a forklift that resulted in days away from work. The employer did not accurately complete column F of the log indicating the description of the injury that occurred.
