29 CFR 1910.147(d)(3):  All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  Instances including, but not limited to the following:  a) Job site, 1080 River Avenue, Pittsburgh, PA: On or about September 3, 2015, during troubleshooting operations at filler line 23A, the employer did not ensure that all energy isolating devices were located and operated prior to the work in order to control power/energy to the unit in that systems such as hot water, product and electrical were not shut off, exposing employees to burn and crushing/amputation hazards.  b) Job site, 1080 River Avenue, Pittsburgh, PA: On or about January 11, 2016, during jam clearing operations at filler line 23A, the employer did not ensure that all energy isolating devices were located and operated prior to the work in order to control power/energy to the unit in that systems such as electrical were not shut off, exposing employees to crushing/amputation hazards.  c) Job site, 1080 River Avenue, Pittsburgh, PA: On or about November 14, 2015, during changeover work at filler line 23A, the employer did not ensure that all energy isolating devices were located and operated prior to the work in order to control power/energy to the unit in that systems such as hot water, product and electrical were not shut off, exposing employees to burn and crushing/amputation hazards.  d) Job site, 1080 River Avenue, Pittsburgh, PA: On or about September 3, 2015, during sanitation/cleaning operations at filler line 23A, the employer did not ensure that all energy isolating devices were located and operated prior to the work in order to control power/energy to the unit in that systems such as hot water, product and electrical were not shut off, exposing employees to burn and crushing/amputation hazards.
