29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or conventional fall protection systems:   On or about September 21, 2015, at the above addressed job site, employees were exposed to a fall hazard of approximately 16.5 feet, while installing tar paper and shingles on a roof without a conventional means of fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about September 21, 2015, at the above addressed job site, employees who had not been provided training to recognize fall hazards were exposed to a fall hazard of about 16.5 feet while installing tarpaper and shingles on a low slope roof of a residence and were standing near the edge of the roof without a means of fall protection.
