29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  On or about February 29, 2016, at the above addressed job site, an employee was exposed to a fall hazard of approximately 18 feet while using the single metal tube of an air handling unit as an anchorage while performing roofing repairs.

29 CFR 1926.502(d)(19): Personal fall arrest systems and components subjected to impact loading were not immediately removed from service and/or were reused before they were inspected by a competent person and determined to be suitable for reuse:  On or about February 29, 2016, at the above addressed job site, an employee was exposed to a fall hazard of approximately 18 feet while using a defective undersized lifeline that had not been removed from service, that had knots and abrasions while performing roofing repairs.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about February 29, 2016, at the above addressed job site, employees were exposed to a fall hazard of approximately 11 feet, while using an extension ladder that did not extend 3 feet above the upper roof landing and lacked a grasping device.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or conventional fall protection systems:  On or about February 29, 2016, at the above addressed job site, employees were exposed to a fall hazard of approximately 18 feet while installing roofing materials from a roof without means of conventional fall protection.  The PAYTON ROOFING, INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1093418, citation number 1, item number 1, and was affirmed as a final order on October 28, 2015, with respect to a workplace located at 13296 NW 5th St. Plantation, FL 33325.  To abate this hazard Payton Roofing, Inc. must provide fall protection to employees engaged in residential construction and exposed to a fall hazard of six feet or more while conducting residential construction.  Date by which violation must be abated:  Immediately upon receipt of the Citation and Notification of Penalty.  Pursuant to 29 CFR 1903.19, Payton Roofing, Inc. must submit documents describing the measures it is taking to ensure compliance with fall protection to eliminate employee exposure to hazardous condition.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about February 29, 2016, at the above addressed job site, employees who had not been provided training to recognize fall hazards were exposed to a fall hazard of about 18 feet while performing roofing repairs, without means of conventional fall protection.  The PAYTON ROOFING, INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1093418, citation number 1, item number 2, and was affirmed as a final order on October 28, 2015, with respect to a workplace located at 13296 NW 5th St. Plantation, FL 33325.
