29 CFR 1910.147(d)(3): All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): On or about September 18, 2015, employee(s) clearing jams on the Comelz Type SS15 Skiver (Tag No. 30965) were doing so and the energy isolating device was not operated as to isolate the energy source. The machine was not unplugged from the power source.
