29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices.     a) Engine Room:  The employer failed to ensure that an eyewash/shower unit was located at the exterior area of the refrigeration room where compressor units, piping and other equipment were located. Employees were exposed to materials such as, but not limited to, ammonia while performing refrigeration maintenance on the ammonia system. In accordance with ANSI/IIAR Standard 2-2008, Section 13.3.1.4 and IIAR Bulletin 109, Section 4.10.10, 1997 edition; OSHA 29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:

(A) The employer did not correct deficiencies in equipment that were outside acceptable limits as required.     a) J & J Snack Foods dba Country Home Bakers: The employer failed to ensure that the ammonia detection system containing sensors remained in an operable condition. The #2 spiral maintained an ammonia sensor that had malfunctioned. Employees were exposed to an inhalation hazard from an exposure to ammonia .

29 CFR 1910.119(l)(2)(ii): The employer did not establish management of change procedures to manage the impact of change on safety and health.         a) J &J Snack Foods dba Country Home Bakers: The employer failed to implement a MOC (Management of Change) protocol  to establish and address the impact of safety and health on employees when assigned duties involving ammonia refrigeration maintenance. The change reflects deviation from normal operating procedures of an enclosed recirculating ammonia system that included spiral #2. Piping on the high side gas line was cut off and altered from the normal defrost pan. Employees were exposed to an inhalation hazard from an exposure to ammonia .
