29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. a) Production area, 1B Conveyor: on or about March 20, 2017, and at times prior, the employer exposed employees to electrical and amputation hazards while performing maintenance on the conveyor 1B belt drive. The lockout/tagout procedures were not specific and di not provide steps for placement of lockout devices. Country Home Bakers was previously cited for serious violations of OSHA standard 1910.147(c)(6)(i), which were contained in OSHA Inspection Number 1099635, Citation 1 Item 3 issued on 04/13/2016 for inspection site located at 720 Metropolitan Parkway Atlanta, GA 30310 and became a final order on 05/03/2016.

29 CFR 1910.147(c)(4)(ii))(C): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. a) Production area, 1B Conveyor: On or about March 20, 2017, and at times prior, the employer exposed employees to electrical and amputation hazards while performing maintenance on a conveyor belt. The lock-out/tag-out procedures were not specific and did not provide steps for placement of lockout devices.
