29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder: a) Jobsite, 395 Federal Street, Pittsburgh, PA: On or about October 6, 2015, during roofing activities, the employer did not ensure that the extension ladder, used to gain access to/egress from the loading dock roof and the ground/parking area, was extended at least 3 feet above the landing surface when extension of the ladder was possible.

29 CFR 1926.503(c): When the employer had reason to believe that any affected employee who has already been trained did not have the understanding and skill required by paragraph (a) of this section, the employer did not retrain each such employee: a) Jobsite, 395 Federal Street, Pittsburgh, PA: On or about October 6, 2015, during roofing activities, the employer did not ensure that employees were retrained in fall protection where deficiencies in fall protection used existed, including, but not limited to, employees working on a roof where warning flags were permitted to lay as low as ground level or where warning flag lines were removed from their location, allowed to lay on roof level and a safety monitor was used where the building exceeded 50 feet in it's least dimension.
