29 CFR 1910.252(a)(1)(ii): When the object to be welded or cut could not be moved and all the fire hazards could not be removed, guards were not used to confine the heat, sparks, and slag to protect the immovable fire hazards: (a) On or about October 19, 2015 and at times prior to that date, for the employees exposed to fire hazards where welding stations, to include but not limited to station number eight (ROU 195) had natural gas utilities exposed to welding slag and other welding/cutting debris in the door installation department at 1820 North Plum Avenue, Brandon, South Dakota. (b) On or about October 19, 2015 and at times prior to that date, for the employees exposed to fire and/or electrical hazards where welding stations, to include but not limited to station number five (ROU 065) had electrical utilities exposed to welding slag and other welding/cutting debris in the door installation department at 1820 North Plum Avenue, Brandon, South Dakota.

29 CFR 1910.305(a)(4)(iv): Open conductors were not separated from contact with walls, floors, wood cross members, or partitions through which they passed, by tubes or bushings of noncombustible, nonabsorbent insulating material: (a) On or about October 19, 2015 and at times prior to that date, for the employees exposed to electrical hazards where electrical conductors, to include but not limited to welding station number six (ROU 066) were in contact with the metal edge of the diamond plate cover in the door installation department at 1820 North Plum Avenue, Brandon, South Dakota.
