The eye solution station in the area had two saline bottles that were opened and not sealed to prevent contamination of the solution. b) On or about October 26, 2015 employees adding DuraPro adhesive to the press line were not provided with an eye-wash station in the immediate vicinity when handling the corrosive substance.
