29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained: At a facility located at 7005 Rickyval Street, Weston, WI 54476; when two employees were conducting cleaning in and around the Mereen-Johnson gang rip saw (Model #435H, S# MJ2773), the employer did not ensure each authorized employee affixed a personal lockout device to a group lockout device, group lockbox or compatible mechanism when they were potentially exposed to caught-between and amputation hazards from unexpected energization of the electrical and pneumatic components of the saw.

29 CFR 1910.213(f)(2): Each self-feed circular ripsaw shall be provided with sectional non-kickback fingers for the full width of the feed rolls. They shall be located in front of the saw and so arranged as to be in continual contact with the wood being fed: At a facility located at 7005 Rickyval Street, Weston, WI 54476; the Mereen-Johnson gang rip saw (Model #435H, S# MJ2773) was not provided with non-kickback fingers while employees used the saw to cut plywood sheets. An employee was exposed to struck-by hazards and was hospitalized due to a piece of plywood that entered the saw and kicked back injuring the employee.

29 CFR 1910.213(f)(1): Feed rolls and saws shall be protected by a hood or guard to prevent the hands of the operator from coming in contact with the in-running rolls at any point. The guard shall be constructed of heavy material, preferably metal, and the bottom of the guard shall come down to within three-eighths inch of the plane formed by the bottom or working surfaces of the feed rolls. This distance (three-eighths inch) may be increased to three-fourths inch, provided the lead edge of the hood is extended to be not less than 5 1/2 inches in front of the nip point between the front roll and the work: At a facility located at 7005 Rickyval Street, Weston, WI 54476; the feed rolls and saw blades of the Mereen-Johnson gang rip saw (Model #435H, S# MJ2773) were not protected by a hood or guard to prevent the hands of saw operators from coming into contact with the in-running nip points of the hold down rolls on the infeed and outfeed side of the saw, or saw blades.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided.
