29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Machine Shop, near door leading to offices, 600 Mayer Street, Bridgeville, PA 15017: Observed on or about October 23, 2015, an electrical outlet box (110 Volt) that was not mounted to the building was equipped with knockouts. The knockouts could be displaced during use. The outlet box was being used to supply power to nearby equipment. Employees using the outlet box were exposed to the hazard of electric shock.

The flexible cable was attached to the wall at various locations along its length.
