29 CFR 1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means were not fully lowered, controls were not netralized, power was not shut off, and brakes set.    On November 4, 2015 employees were exposed to the hazards associated with working around a Pettibone rough terrain fork truck that was left unattended with the load engaging means in the elevated position.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.502(d)(6)(i):   Unless the snaphook is a locking type and designed for the following connections, snaphooks shall not be engaged directly to webbing, rope or wire rope.     On November 4, 2015 rope lifelines with snaphooks were wrapped around a 2"x4" truss members and attached back to themselves, exposing the employees to fall hazards up to 20.2 feet due to unintentional disengagement or breakage of the lifeline.        Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  On November 4, 2015 anchorage points for personal fall arrest systems were not capable of supporting 5,000 pounds, exposing employees to the hazards associated with utilizing a personal fall arrest system that did not maintain a safety factor of at least two.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       On November 4, 2014 an employee was not protected from a fall while performing residential construction activities, exposing the employee to a fall of 20.2 feet.      Timberline General Contractors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1014822, citation number 1, item number 1 and was affirmed as a final order on March 5, 2015, with respect to a workplace located at Eagle Cr. and Anderson Dr. New Lenox, Illinois 60451.       Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
