29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall:    a) Facility, 11 Lloyd Avenue, Latrobe, PA:  On or about November 17, 2015, employees were exposed to potential hazards because the employer did not complete a workplace PPE assessment at the new location to determine what work activities needed PPE and what type of PPE would protect the employees during those activities for new equipment such as, new PDC sleeve labeler, new sleeve seal labeler, and carbonator and existing (from the previous location) equipment such as the packer lines.  Employees are handling glass bottles that can and break/explode therefore, requiring gloves.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    (a) Facility, 11 Lloyd Avenue, Latrobe, PA:  On or about December 10, 2015, visual inspections of portable cord and plug connected equipment and flexible cords were not conducted before use for external defects such as the 25 foot extension cord that had black tape on it to cover the defect, that was being used to power the grinder to perform maintenace on the packer machine.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a) Facility, 11 Lloyd Avenue, Latrobe, PA:  On or about December, 10, 2015, an employee was exposed to electrical hazards while using an extension cord that did not have adequate strain relief and the outer jacket was pulling away from the plug end.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Facility, 11 Lloyd Avenue, Latrobe, PA:  On or about November 17, 2015, employees are exposed to fly chips while operating the Hartness International , Inc. model number 825 Packer on the carb line, that was not fully guarded to protect the operator.    Castle Co-Packers, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 895679, citation number 2, item number 1 and was affirmed as a final order on July 31, 2013, with respect to a workplace located at 204 B Schreiber Industrial Park, New Kensington, PA.
