29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: a) Jobsite, D-Pile Area, 11 Lloyd Avenue, Latrobe, PA 15650: Observed on or about July 18, 2022, the only exit route from the mezzanine level in the D-Pack Area was not kept free and unobstructed. The storage of pallets, boxes, equipment and other items in the D-Pack area restricted exit access from the mezzanine area. Employees working in the mezzanine area did not have free and unobstructed exit routes in the event of an emergency.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage: a) Jobsite, D-Pile Area, 11 Lloyd Avenue, Latrobe, PA 15650: Observed on or about July 18, 2022, empty pallets were stacked directly in front of an electrical panel (480 volts, 3 phase power) that had a yellow label that read '1E1.' Access to this panel was completely blocked by the pallets. b) Jobsite, D-Pile Area, 11 Lloyd Avenue, Latrobe, PA 15650: Observed on or about July 18, 2022, a pallet and two drums of solution were stored in front of an electrical panel identified as 'Control Cabinet #6.' Access to this panel was restricted by the storage of materials in front of the electrical control cabinet.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a) Jobsite, D-Pile Area, 11 Lloyd Avenue, Latrobe, PA 15650: Observed on or about July 18, 2022, an electrical (480 volt, 3 phase) panel identified as 'Control Cabinet #6' was left open and plastic was draped over a portion of equipment on the inside (upper left side) of the cabinet. The cabinet housed 480 volt/ 3 phase conductors. Employees working in the area of the cabinet were exposed to electrical shock hazards and fire hazards.
