One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a. Located in the Die Mold Department at 420 East High Street in London, Ohio: On or about November 9, 2015 and at times prior, the employer did not ensure that employees who operate a horizontal band saw manufactured by W.F. Wells, Model W-9-1, Serial W117441, were protected from incidental startup of the machine while making minor adjustments and from unused portions of the cutting blade which were not properly guarded, thereby exposing employees to an amputation hazard.
