29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a. On or about June 16, 18, 19, 20 and 22, 2018, at the work site located at 420 East High Street, London, Ohio, employees used a platform on a fork lift truck to perform die changes in Machine #58. The platform had not been approved for use by the fork lift manufacturer.

29 CFR 1910.217(d)(9)(iii): The employer shall provide die stops or other means to prevent losing control of the die while setting or removing dies in presses which are inclined. a. At the work site located at 420 East High Street in London, Ohio, employees used a fork lift with a platform to change the dies in Machine #58 for 9 times while the die cart was broken. The platform had die stops that were not utilized to prevent losing control of the die during the removal. On the 10th change, an employee was injured when the 4,800 pound die did not stop, falling off the platform and pinning the employee to the floor under it.
