29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a) On or about November 19, 2015 at worksite 321 NW 96th BLDG A, Kansas  City, Missouri.-employees were exposed to a fall hazard when performing roofing activities at an elevation of approximately 31 feet without an approved form of fall protection, to include, guardrail systems, safety net systems, or personal fall arrest systems.     Abatement Note: Abatement certification for this violation is not required.

29 CFR 1926.503(a)(2)(iii):   The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.   (a) On or about November 19, 2015 at worksite 311 96th St BLDG A, Kansas City, Missouri.- employees engaged in framing activities on a low-sloped roof with unprotected sides and edges were exposed to serious fall hazards, in that they had not been trained in the use and operation of required Fall Protection Systems.  As result, employees were exposed to fall hazards  approximately 31 feet above the  surface below.     Abatement certification is required for this item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1926.1053(b)(1):    When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder:    (a) On or about November 19, 2015 at worksite 321 NW 96Th St BLDG A, Kansas City, Missouri.-employees were accessing a working surface on a low pitch roof approximately 31 feet in height by way of a portable ladder that did not extend at least 3 feet above the upper landing surface being accessed.    Abatement Note: Abatement certification for this violation is not required.
