29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: COMPRESSED GAS STORAGE AREA: On or about November 30, 2015, compressed gas cylinders including; but not limited to oxygen, argon, and nitrogen; were stored outside where they were exposed to the elements of nature, including prolonged dampness. In addition, the cylinders were standing directly on the ground with nothing to protect them from rusting. (REF: CGA Pamphlet P-1-1965, Sections 3.3.7 and 3.3.9)

29 CFR 1910.102(a): 29 CFR 1910.102(a) In-plant transfer, handling, storage, and/or use of acetylene in cylinders did not comply with the provisions of CGA Pamphlet G-1-2009 ("Acetylene"): COMPRESSED GAS STORAGE AREA: On or about November 30, 2015, compressed gas cylinders containing acetylene were not stored in accordance with Section 5.2 of CGA Pamphlet G-1-2009 in that: A) Three cylinders were not adequately segregated from oxygen cylinders. The acetylene cylinders were stored 12 inches in front of the fire wall, outside of the designated storage area. B) The cylinders were stored in an unprotected area outside. The cylinders were standing directly on the concrete pavement. In the winter, the cylinders were not protected from the accumulation of ice and snow.

29 CFR 1910.1200(e)(1)(i): The employer did not compile a list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet. SITE-WIDE: On or about November 30, 2015, no chemical inventory was included as part of the Hazard Communication Program. Hazardous chemicals that employees may be exposed to on site include, but are not limited to: compressed gases (acetylene, oxygen), flammable paints and solvents, and hexavalent chromium.
