29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    A). On or about December 15, 2015 employees were utilizing three flexible extension cords to energize power tools to cut roof sheathing to size and were not protected from electrical shock hazards because the path to ground was interrupted because the ground pin conductor had been broken off/removed.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a). On or about December 15, 2015 employees were utilizing two Werner brand aluminum extension ladders to access the roof section of a new one story block commercial structure and were not protected from slip and fall hazards while stepping onto or off of the ladders because the ladder side rails did not extend at least 3 feet above the landing surface being accessed.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system.  A). On or About December 15, 2015 employees engaged in decking the roof on a one story commercial structure are not protected from falls by any means of a safety net, guardrail system, or personal fall protection system exposing the employees to falls in excess of 12 feet to the ground below.   Florida's Frame Pro, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 315480699, citation number 1, item number 1, and was affirmed as a final order on November 11, 2011, with respect to a workplace located at 13162 Atlantic Blvd, Jacksonville, FL 32225.
