29 CFR 1926.502(f)(4): Mechanical equipment on roofs were used and stored in areas where employees were not protected by a warning line system, guardrail system, or personal fall arrest system:    a) On or about December 15, 2015, employees were exposed to fall hazards where mechanical equipment was used and stored in areas not protected by a warning line system, guardrail system, or a personal fall arrest system.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      a)  On or about December 15, 2015, employees were removing a debris chute and were exposed to fall hazards from the roof of a three story building.      Star Roofing and Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 190631, citation number 1, item number 1 and was affirmed as a final order on March 29, 2012, with respect to a workplace located at the N/E corner of S. West Ave. and St. Charles Rd., Elmhurst, IL, 60126.    Star Roofing and Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 922347, citation number 2, item number 2 and was affirmed as a final order on April 25, 2014, with respect to a workplace located at 3201 W. Fullerton Ave., Chicago, IL, 60647.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

Star Roofing and Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(3), which was contained in OSHA inspection number 749421, citation number 1, item number 3 and was affirmed as a final order on January 24, 2013, with respect to a workplace located at 1943 N. Larrabee St., Chicago, IL, 60614.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:      a)  On or about December 15, 2015, employees engaged in roofing activities were exposed to falls from the roof of a three story building.    Star Roofing and Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 315508606, citation number 1, item number 1 and was affirmed as a final order on July 5, 2011, with respect to a workplace located at 400 E. Golf Rd., Arlington Heights, IL, 60647.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
