29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (6): a) On or about December 14, 2015 employees were exposed to machine hazards associated with crushing and thermal hazards while they removed and installed machine components on the Redford 22 core machine while it was energized. The employer failed to ensure that employees implemented energy control application steps including, but not limited to: (1) turning off and shut down the machine, (2) physically locating and operating energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (3) affixing lockout or tagout devices to each energy isolating device. (4) prior to starting work on machines that have been locked out or tagged out, verifying that isolation and deenergization of the machine or equipment had been accomplished.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about December 22 the two sides and the back of the Redford 22 core machine was not guarded to prevent employee exposure to moving machine components including but not limited to the cradle, head plates and die box halves movement. b) On or about December 22, 2015 the crushing hazards created by the downward movement of the four cradle guide rods was not guarded to prevent employee exposure to the hazardous area.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about December 22, 2015 an eight inch portion of the light curtain installed on the ATD Redford 22 core machine was obstructed by a gray material. The modified light curtain did not prevent employee exposure to the hazardous downward motion of the die box. Alliance Industries, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number 312560477, citation number 1, item number 3 and was affirmed as a final order on June 13, 2011, with respect to a workplace located at N2467 Vaughan Road, Waupaca, WI 54981.
