29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about January 6, 2016:  Employees for Sergio Garcia were exposed to fall hazards:  a) Two employees installing floor sheathing were exposed to fall hazards of approximately 40 feet when the employer failed to ensure they were properly tied off while preforming tasks.  b) One employee and a foreman were exposed to falls of approximately 10.3 feet when the employer failed to ensure that the employees were keeping tie off points and lanyards shorter than fall hazard.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
