29 CFR 1910.305(j)(1)(i): Fixtures, lampholders, lamps, rosettes, and receptacles located had live parts normally exposed to employee contact. Anderson terminal, outside sandblasting booth: On or about 7 January 2016 and times prior thereto, a 120 electrical outlet cover was missing on maintenance vehicle 16-40. Vehicle was plugged into shore power. This condition exposed employees to an electrical shock and burn hazard.

Anderson Terminal, Maintenance shop: On or about 7 January 2016 and times prior thereto, the Standard Modern Lathe, serial number 3104, was missing a chuck guard. This condition exposed employees to a caught in and struck by hazard.

29 CFR 1917.152(e)(2)(ii): Arc welding or cutting cable was used when it was not free from repair or splice for 10 feet (3 m) from the electrode holder. (a) Anderson Terminal, Maintenance shop: On or about 7 January 2016 and time prior thereto, the stinger lead on the Miller XMT 456 CC/CV welder had broken insulation approximately two feet back from the stinger. This condition exposed employees to an electrical shock and burn hazard. (b) Anderson Terminal, Maintenance shop: On or about 7 January 2016 and time prior thereto, the stinger lead on the Thermal Dynamics Cutmaster 82, serial number 04885740 had broken insulation. This condition exposed employees to an electrical shock and burn hazard.
