29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) KRM Machine - An employee checking the quality of tubing had four (4) fingers and the thumb of his right hand partially amputated when he was pulled into the unguarded in running nip point of the KRM Machine, on or about November 10th, 2015.   b) Maintenance Horizontal Band Saw - Employees cutting quarter (1/4) inch metal stock were exposed to cutting hazards from a guard that did not adjust under eight (8) and a half (1/2) inches, on or about November 13th, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
